Intrauterine insemination – preparation and course of the procedure

Intrauterine insemination is one of the basic techniques of assisted reproduction and expensive IUI Treatment Cost in Pakistan. It is a safe method of treating infertility that has been widely used for many years.

What is Insemination?

It consists of the direct injection of previously donated semen into the woman’s uterus, without having direct sexual intercourse. This is to bypass cervical mucus and antibodies that prevent sperm from reaching the uterus during intercourse. In addition, before administration, the semen is subjected to laboratory procedures aimed at selecting sperm with the highest mobility.

Indications for intrauterine insemination:

  • slightly abnormal semen parameters: too low number and insufficient sperm motility,
  • ejaculation disorders,
  • the presence of anti-sperm antibodies,
  • stage I/II endometriosis (growth of the endometrium outside the uterus),
  • Unexplained cause of infertility.

Before insemination and qualification for treatment, the patient should have the patency of the fallopian tubes verified. When preparing for insemination, it is necessary to monitor ovulation with ultrasound. Ovulation stimulation improves outcomes in couples with unexplained infertility and endometriosis. The purpose of ovulation stimulation is to increase the chance of pregnancy by getting 1-3 mature follicles.

Preparing a man for Insemination

Before donating semen for insemination, it is necessary to maintain a 2-7 day period of sexual abstinence. Semen is donated in specially prepared rooms. Currently, unfortunately, we do not have any evidence that any pharmacological substances can improve semen parameters.

Types of Insemination

  • Intrauterine insemination with the sperm of the husband/partner AIH
  • Intrauterine insemination with AID donor sperm**

Insemination with donor sperm

In a situation where insemination with the husband’s/partner’s sperm is not possible, for example, due to the low quality of semen, lack of semen, or carriers of certain genetic defects in partners, insemination is possible with the use of semen from an anonymous donor.

To select the right donor, the biologist conducts an initial interview with the couple.

The interview includes information such as:

  • blood group in the AB0 system,
  • growth,
  • eye color,
  • hair color,
  • Physique.

All donors whose semen is used for AID intrauterine insemination are rigorously screened for:

  • infectious diseases, i.e. HIV, HBV, HCV,
  • bacterial diseases, i.e. chlamydiosis and syphilis,
  • mutation of the CFTR gene, i.e. a gene located on the long arm of chromosome 7 at the 7q31.2 locus, the incorrect form of which can cause a genetic disease called cystic fibrosis,
  • karyotype tests, i.e. quantitative and qualitative analysis of chromosomes.

The course of the procedure

Preparation for insemination

1. Medical visit

Based on the test results obtained from IUI Treatment Lahore, the doctor qualifies for the insemination procedure and starts monitoring the patient’s cycle or decides on the procedure based on the patient’s natural menstrual cycle, and asks for reading and signing the necessary consents in his presence.

If the couple is not married, the man joining the insemination procedure with the use of donor semen with his partner will have to sign a statement at the Registry Office that he will be the dad of the child. Within 3 months from the date of the man’s declaration, the partner will have to confirm at the Registry Office that the man will be the father of the child. If the man does not submit a statement to the Registrar’s Office, the couple will not be able to proceed with the insemination procedure. 

2. It is also necessary to monitor the course of the cycle to determine the day of ovulation.

The course of the insemination procedure

1. On the day of the procedure, you will be asked to report to the center at the agreed time.

2. The first stage of the procedure is the donation of semen, which is sent to the laboratory. The semen preparation process takes up to hours.

3. The insemination procedure is performed in the office on the gynecological chair.

4. Using a thin catheter, the sperm is inserted into the uterine cavity. The insemination procedure does not require anesthesia.

5. The procedure is painless and the woman can go home immediately after it is performed.
